Oak & Iron Modern Skin Studio (“Oak & Iron”) is a skin studio in North Myrtle Beach, South Carolina. This policy covers the website and the Oak & Iron app.

What we collect

When you book, you give us the details needed for the visit: your name, your email, your phone number if you choose to add it, the treatment, and the time you choose. We use that only to hold the chair, reach you about your visit, and care for you in studio. Your phone number is optional and is used only so Laura can contact you about your appointment.

Skin analysis & consent (health information)

When you book a facial, dermaplaning, a chemical peel, microneedling or a full-face wax, we ask you to complete a Skin analysis & consent form before your booking is confirmed. It collects:

• your name, email, optional mobile number and date of birth;
• an emergency contact (name and phone), and for clients under 18, a parent or guardian’s name, phone and relationship;
• skin and health answers: skin type and sensitivity, sun response, concerns and goals, skin products you use, medications (including isotretinoin), allergies, pregnancy or nursing, health and skin conditions, recent sun exposure and recent treatments (waxing, injectables, laser, peels, surgery), and past reactions;
• your consent choices (including optional before/after photo choices), your typed name and drawn signature, and for minors the parent or guardian’s signature.

Why: we use this information only to decide whether a treatment is safe for you and to care for your skin.

Who can see it: only you (through the secure link for your visit) and Laura (through the studio’s owner sign-in). Each time Laura opens a form, the view is recorded in an access log.

How it’s protected: your answers and signature are encrypted before they are stored, with a key held only by the studio’s server, and they are sent over secure (HTTPS) connections. Our hosting provider (Cloudflare) stores the encrypted data. Emails and notifications about your form never include your answers; they only say that a form is due or was received and give you a secure link.

Returning visits: after your first full form we keep it on file under your email. At later visits we ask “What changes have occurred?” If you choose None, we record your new signature and the time. If you choose Some changes, we ask only about what changed and keep a record of the changes for Laura.

No sale or sharing: we don’t sell this information, use it for advertising, or share it, except with your permission, to protect your safety in an emergency, or when required by law.

How long we keep it: 5 years after your last visit, then we delete it. Ask us any time for a copy or to correct your information.

When you sign the form electronically we record your typed name, your drawn signature, the date and time, the form version, and basic technical details of the device you signed on (IP address and browser or app version) so the signature can be verified.

Payment

Payment for visits stays at the studio for now. We do not sell in-app purchases. We do not store full card numbers on our own servers.

How we use it

We use your information to confirm bookings, send appointment and Skin analysis & consent reminders by email, follow up on a visit, and run the studio. We do not sell it. We do not use it for unrelated advertising.

Keeping it

We keep booking records as long as they are needed for your visits and for ordinary studio records, then we let them go. Skin analysis & consent forms and signatures are kept for 5 years after your last visit, then deleted.

Your choices

Ask us for a copy of your Skin analysis & consent form, or to correct or remove details that are no longer needed (intake records are kept for the period above). Cancellation: Please cancel at least 24 hours before your appointment. Cancellations inside 24 hours are subject to a fee of 50% of your booked services (payable at the studio), as described on the site and in the app.
